Teensy3.1 Got Heated and stopped working

Hi,

Teensy3.1 IC got heated and the LED faded during blink (as part of s/w) .

The following observations are observed.
1. The controller is provided with 5V DC externally.
2. GSM modem is connected to serial port no.1
3. GPS receiver connected to serial port no.2
4. Modbus slave connected to serial port no.3 via Max485

All the above interfaces have 5V and analog i/p are with 3.3 V.
We have connected the mini USB connector to my laptop to view the serial window for debugging purpose. We have faced this situation for 3 controllers, kindly suggest us what would have gone wrong and provide us the solution for not making the mistake again.

We are providing 5V to Vin pin which mentions 3.7V to 5.5 V as the input voltage.
However we are taking care to see that IO pins does not exceed 3.3V.
 
So you connected 5vdc to Vin pin and not a 3.3 volt pin? did you connect multiple supplies or grounds that could cause looping EX: bench power supply and usb cable at same time? Is your bench power supply input gnd isolated from its output? (i wreked some stuff before doing that).
 
To add to the above; I fixed my issues by isolating all the supplies gnd's then tying all the gnd's through high voltage ceramic caps to a common earth ground
