I'm thinking along the lines of PC USB issue combined with the draw of particular cards might just tip the scales. I ordered a 48W capable USB PCIE x16 USB host card to see if the SD cards show the same behavior with that as the controller/power.

Top 3 cards all had the issue, bottom Samsung was fine

ohhh man your right, I dug up a Samsung card and it did not happen, Sandisk, SP and TK (whatever that is) they all failed but the Samsung worked. Thank you

Regular SDHC, no adapter, I tried a few different cards just to make sure it wasn't the card, no luck

I have gone over and over my code, tried several 4.1 boards but they all reboot whenever I remove then insert an SD card after initialization. Im trying to make sure I have hot-plug SD support but so far only seems to work if the card is present...
