DOA Teensy 4.1?

I just received my Teensy 4.1-NE (without Ethernet) and it seems to be DOA. When plugged into a USB cable there's not any lights, I expected a blinking light.

I've measured +5 on the VIN pin, however, the 3.3V pin shows 0.5mV. There's continuity between the GND pins.

I'm wondering if the board came with a bad voltage regulator.

I bought the board from Sparkfun if that helps.

Unless you've connected stuff to the on/off pin which could cause it to turn off, that does sound like a bad one.

The 2 tests they are supposed to run on every board absolutely do test the voltage regulator. Neither test can pass if the board doesn't power up. Difficult to know what really happened, like damage in handling / shipping?
 
I went and connected 3.3V to the pin and the board works. On closer examination of the board, it looks like the USB connecter is canted some, is it enough to cause this issue, I'm not sure. I'll have to cut up a USB cable and see
