Many things I don't understand about this schematic. Can't really offer advice.
For example, I don't know which part is Teensy. Maybe J2 is meant to be half of Teensy 4.1. I see a similar J3 connector, but only 20 pins. Likewise for the other connectors, other than MIDI IN and MIDI OUT, impossible to know what will be connected.
I can at least comment the R3 and R4 resistors need to be lower. 220 ohms is for 5 volt signals. Look at the recommended schematics on this page for details.
Also pin 6 on U2 (optocoupler) looks unconnected. It's output can't work without power.
That's as much as I can help without a clearer schematic that actually shows or documents what will really be connected.
