Teensy 4.1 Dead or not?

Hi everyone,

I have a Teensy 4.1 that I can't get Teensy Loader to recognize consistently.
I've done the 13 second button hold for the factory reset, the red light turns on, I let go, and it stays lit for a while and turns off.
However, upon connecting this to my Mac I'm unable to get it to recognize the Teensy.
I also don't see the orange light blinking - I've heard that the factory reset should re-flash the Blink code to the Teensy but that doesn't seem to be working.

I don't see any stray solder pieces on the Teensy ( I hadn't soldered anything to it anyway), the parts look fine, nothing looks chipped, etc.
I'm pretty stuck as to what might be the issue here. Has anyone seen this before?
 
After the factory reset, the Teensy is in a mode where it doesn't show up as a serial device until you flash a new program. Try that first with one of the simple examples or something. The Teensy Loader program should see it.

I'm not certain why blinking isn't happening after that reset, however. Is this a locked lockable Teensy? I don't think the blink happens after a factory reset for those.

Just to confirm, you are seeing the quick LED blink at ~13 seconds of hold the button, right?

When you release the button just after that 13 quick blink, how long does the red LED stay on? It should be more than 1 minute on Teensy 4.1. The erase process is quite slow.

When it finishes, the red LED (near USB connector) is supposed to turn off and the orange LED (near pushbutton) is supposed to slowly blink. Arduino IDE should detect the device as RawHID, not serial.
