Teensy 3.2 USB

I’m attempting a music project with a Teensy 3.2 enclosed in a box, using a USB breakout board (see png). Unfortunately, with this wiring, I can’t upload a new sketch. I get the following error message: “Teensy did not respond to a USB-based request to automatically reboot. Please press the PROGRAM MODE BUTTON on your Teensy to upload your sketch.” I have tried this with no success.

Two possible reasons:
A) Your sketch is not working and crashes - this will stop the usb code,too.
- You can use the "Program" Pin, and add a momentary switch to box

B) The adapter does not work.
