Teensy 3.5 Goes dead in production? Needed programming, how to mitigate?

I have a product that uses the 3.5 as a bootstrap/platform. It uses the Teensy 3.5 as a USB audio device. Now what happened was the customer complained that the device has stopped functioning. Doesn't make USB sound, no lights, and nothing pops up in device settings.

I got around to sending him the hex file and allowing him to flash it, and that worked.
That same morning, while testing two of my devices, one of them did the same thing - just randomly like that. I only moved from one computer to another.

So my questions are:
  • what could have caused this?
  • did it lose its program or did something got corrupted on the teensy or PC?
  • how do I prevent this from happening?
