What is the pad near the ground? Is it 2 ground pins next to each other?

What about the 5 pads near the SD-Card? Are they the SD-Card signals access?

The interior pad near USB connector is NOT GND. That is the VUSB pin - i.e. 5 volts from USB unless the trace is cut or power is applied externally to the VIN pin.

If you have any other ARM Teensy card look at the back of that card - they are all posted on PJRC.com as well :: pjrc.com/teensy/pinout.html

That T_4.0 image is most telling it also shows the other 5 pins in a row and details their function. They are shifted from T_4.0 to T_4.1 - but the usage is the same. :: pjrc.com/teensy/card10b_rev1_web.pdf

Other exterior pins are the same where present - but the bottom pads are either gone or have different function - for instance the USB Host pads on T_4.0 migrated to the same offset spacing for USB Host on the T_3.6, and those two pads on T_4.1 are instead the Device USB D+/D- pads. And the T_4.1 added two memory pad arrays under the SD card socket.
